Netgear AGM721F: ProSafe 1000BASE-SX GBIC module, 850nm multimode, SC connector.

The Netgear AGM721F is a ProSafe gigabit interface converter providing a 1000BASE-SX fibre port, transmitting at 850nm over multimode fibre through a duplex SC connector and reaching up to 550 metres.

It is hot swappable and complies with IEEE 802.3z, and Netgear lists it as the fibre option for the FSM750S, FSM726S and FSM726 switches, which is exactly the job it does: turning a GBIC slot on a Fast Ethernet managed switch into a gigabit fibre uplink for a run between floors or buildings. Both 62.5 micron and 50 micron multimode cabling are supported, with the longer reach available on the 50 micron grade.

This is the older full size GBIC form factor rather than the smaller SFP, so check which cage your switch presents before ordering.
