The Polycom 1465-42340-002 is an AC to DC power supply unit, referenced as the SPS-12A-015 and rated at 24 volts and 0.5 amps. Polycom used external adapters of this class across its desk phone and conference phone accessories, where the console or interface module draws too little current to justify an internal supply.

Output voltage and polarity matter more than the badge on a replacement, and at 24 volts this unit is not interchangeable with the 48 volt or 5 volt adapters used elsewhere in the range, so the part number rather than the physical connector is the thing to match.
